AMENDMENT 0004: The purpose of this amendment is to: Update the Performance Work Statement Update the Price/Cost Schedule Update the Clauses and Provisions All responses due no later than 10:00 AM MT 08/05/2026 via email only to Katharine Robert, at [email protected] . See the attached solicitation for submission instructions and required submittals under FAR 52.212-1, and evaluation factors under FAR 52.212-2. This is a solicitation for commercial products and commercial services. The Government contemplates a single award of a base plus four option year, Fixed-Price Indefinite Quantity contract resulting from this solicitation. This is a 100% SDVOSB set-aside solicitation for Non-Emergent Medical Transportation (NEMT) Services for the Phoenix VA Healthcare System (PVAHCS).
