Amendment 0001 is hereby issued to add to: 1)H.16 - Quality Assurance Plan (QAP), 2) Attachment 11- Quality Assurance Plan 3) 52.222 - 90 - Addressing DEI Discrimination by Federal Contractors (APR 2026). The attached questions and answers regarding the requirment are also added. All other Terms, Conditions, and Line Items remain the same. Solicitation number 15BFA026R00000003 is issued as a Request for proposal (RFP) for Comprehensive Medical Services. This acquisition is not set-aside for small business concerns. (iii) No brand name requirements for the items are to be acquired.

(iv) Services rendered will be provided at FMC Fort Worth located in Fort Worth, Texas. The date and times will be determined after award. Please see attached solicitation for details.
